HealthCheck provides a platform for booking health check appointments with certified healthcare providers. Our services include appointment scheduling, health assessments, preventive screenings, diagnostic testing, and specialist consultations. We act as an intermediary to connect patients with qualified medical professionals.
User Responsibilities
As a user, you agree to:
- Provide accurate and complete information when booking appointments and using our services.
- Arrive on time for scheduled appointments or provide adequate notice for cancellations.
- Follow pre-appointment instructions provided by our team.
- Not use our services for any unlawful or unauthorized purpose.
- Keep your account information and communications secure.
- Respect the healthcare providers and staff during your interactions.
Appointment Policies
- Booking: Appointments can be booked through our website. You will receive a confirmation email upon successful booking.
- Cancellation: Appointments must be cancelled at least 24 hours in advance. Late cancellations or no-shows may be subject to a cancellation fee.
- Rescheduling: Appointments can be rescheduled subject to availability. Please contact us at least 24 hours before your original appointment.
- Late Arrival: If you arrive more than 15 minutes late, your appointment may need to be rescheduled.
Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept major credit cards, debit cards, and select insurance plans. For insurance claims, patients are responsible for any co-pays, deductibles, or services not covered by their plan.

- We do not guarantee specific health outcomes or results from any services provided.
- HealthCheck is not a substitute for emergency medical care. In case of a medical emergency, please call 911 or visit your nearest emergency room.
- We are not liable for any damages arising from the use or inability to use our services, except where required by law.
- Third-party services integrated into our platform have their own terms and conditions.
Intellectual Property
All content on the HealthCheck website, including text, graphics, logos, and software, is the property of Broad Leads LLC and is protected by intellectual property laws. You may not reproduce, distribute, or create derivative works from our content without prior written consent.
Dispute Resolution
Any disputes arising from these Terms and Conditions or your use of our services shall be resolved through good-faith negotiation. If a resolution cannot be reached, disputes will be submitted to binding arbitration in accordance with the rules of the American Arbitration Association, conducted in the State of Florida. Both parties agree that any arbitration shall be conducted on an individual basis and not as a class action.
